You may apply anytime between November 1, 2012 and the April 30, 2013 unless funding is exhausted before then.

Residents of Davis, Morgan, Washington, or Weber counties may apply until September 30, 2012 if funding is available.

Applicants have two ways to apply:

1. Click on the Local HEAT offices tab and call the office in your county to schedule an appointment.

OR

2. Download an application above, completely fill it out, and send it in. Read the instructions first! Mailed applications lacking proper information will not be processed.

Call the UTAP Program at 1-800-948-7540 for an application

Ask your phone provider to send an application

2. Fill it out completely, then mail or fax it the UTAP Program with any required documents.
