New Choices
The New Choices Program helps people live at home rather than in a nursing
home. The services given include:
help with cooking,cleaning and other daily activities,home health services,help with taking your medicine,and case management.
If needed, customers can receive limited nursing home care if it doesn't go
over 90 days at a time.
How to Qualify:
You must be age 65 or older, or be age 21 through 64
and considered disabled by Social Security or the State Medicaid Disability
Review Office.You must be eligible for Medicaid coverage group, such as SSI recipients,
SSI Protected Group individuals (such as Adult Disabled Child, Disabled
Widows/Widowers, Medicaid Work Incentive, etc.).
