New Choices

The New Choices Program helps people live at home rather than in a nursing home.  The services given include:

  • help with cooking,
  • cleaning and other daily activities,
  • home health services,
  • help with taking your medicine,
  • and case management.
  • If needed, customers can receive limited nursing home care if it doesn't go over 90 days at a time.

    How to Qualify:

  • You must be age 65 or older, or be age 21 through 64 and considered disabled by Social Security or the State Medicaid Disability Review Office.
  • You must be eligible for Medicaid coverage group, such as SSI recipients, SSI Protected Group individuals (such as Adult Disabled Child, Disabled Widows/Widowers, Medicaid Work Incentive, etc.).
