Basic Education Training Provider Application

 If your school, or you as an individual youth tutor, would like to become an approved training provider of basic education, you’re in the right place.

How to Become an Approved Basic Education Training Provider:

  • Your school must have been in business in Utah for at least one year from the date you apply for approval. (Not a requirement for youth tutors or public schools governed by the Utah State Office of Education).
  • Determine which application type matches your school.
    • Are you a public school governed by the Utah State Office of Education?
      Application A
    • Are you an individual, not part of a business, wanting to provide tutoring in basic education subjects to youth aged 14-21?
      Application B
    • Are you a private school offering Adult High School/GED Preparation, English as a Second Language or other Basic Education?

  • Download and complete the application.
  • Mail the completed application with the required documents to:

The application process may take 90 days or more to complete. DWS cannot pay for any costs incurred prior to approval.

Upon Approval

 You will be required to prominently post the DWS Equal Opportunity Notice. Placement of your school or organization’s information on our Basic Education Approved Training Provider list will occur. DWS customers will choose the school that best meets their needs and contact you if chosen. If you have questions, please email dws_trainingproviders@utah.gov.
