The Utah State Office of Rehabilitation in Roosevelt will be moving into a new location at 140 West 425 South #330 starting Monday, July 23, 2018. 

The office is moving to be co-located with its Workforce Services peers in the Employment Center. The co-location will provide a variety of additional resources and services to clients from this new location.

The Utah State Office of Rehabilitation in Payson will be moving into a new location at 1185 North Canyon Creek Parkway in Spanish Fork starting Monday, July 9, 2018.

The office is moving to be co-located with its Workforce Services peers in the Employment Center. The co-location will provide a variety of additional resources and services to clients from this new location and is convenient for public transit.
