FAQ - Primary Care Network (PCN)

Who can apply?

  • Adults age 19-64 who cannot get other health insurance and are not eligible for Medicaid
  • What does PCN cover?

  • Visits to a primary doctor, 4 prescriptions a month, dental exams/cleaning/fillings/x-rays, eye exams, etc.  See the PCN Member Guide for more information.

  • Do I have to pay for PCN?

  • The yearly enrollment fee is $50 or less (depending on your income or if you receive GA).  There are also small co-payments for certain services.
  • When is the open enrollment for PCN?

  • You can only apply for PCN during an open enrollment period.  The Department of Health and the Department of Workforce Services advertises when an open enrollment period is.
  • Which doctors will take PCN?

  • To get more information about your benefits and which doctors accept PCN, talk with your local Health Program Representative (HPR).  You can find your local HPR by calling toll free 1-866-772-1261 or going to http://health.utah.gov/umb/
