Appeals Forms
If you wish to appeal an INITIAL Department of Workforces Services decision concerning Unemployment Insurance, click on "Initial Appeal" below and provide the requested information. Make sure to include why you believe the decision is incorrect and the relief you are requesting. A copy of your appeal will be sent to any other interested parties.
If you wish to appeal the decision of an Administrative Law Judge (ALJ) to the Workforce Appeals Board, click on "Appeal to the Workforce Appeals Board" below and provide the requested information. You must state the grounds upon which the appeal is being made. If your appeal has not been filed within 30 days from the date of the Administrative Law Judge’s (ALJ’s) decision, you will need to explain why your request is late.
