Reporting Requirements
Every employer is required to report seven mandatory data elements within 20
days of any newly hired/rehired employee's first day of work.
Employee
- Name
- Social Security Number
- Mailing Address
- Date of Hire or Rehire
- Date of Birth *
Employer
- Name
- Federal Employer Identification Number
- Mailing Address
* We also encourage employers to report Date of Birth optional
data element
