  • For quarter end 6/30/2021, the Form 66 (Statement of Employment Benefit Costs) was sent to some employers in error. The affected employers did not have unemployment benefit charges for the quarter. How to identify the form sent in error: The “Calculation of Benefit Costs” section that shows the claimant’s name will be blank. Please disregard the blank notice since no charges occurred during the quarter.
