Department of Workforce Services Privacy Notice
This Privacy Notice describes how the Department of Workforce Services (“ Workforce Services”) handles personal information that you provide to us, what is disclosed, with whom we share your information, and how you can request access to this information under the Government Data Privacy Act (“GDPA”), Utah Code Title 63A, Chapter 19. In addition, there may be more restrictive state and federal laws that apply.

What Personal Data do you collect?
Workforce Services collects personal data from individuals, that may include:
- Names, such as full names, children’s names, parents or guardians names;
- Contact information, such as home or mailing addresses, phone numbers, pager numbers, and email addresses;
- Verification of identity, such as birth certificate, date of birth, place of birth, Social Security numbers; identification numbers; physical identifying characteristics; or biometrics; driver’s license; passport; or registration cards;
- Family status: single or marital status; or family relationships;
- Identifying characteristics or information, such as IP addresses; photograph; religion; languages spoken; race/gender/ethnicity; military service; banking information; citizenship status; criminal record; background checks;
- Income records, such as income, wages, or tax records;
- Medical and health information, such as, health insurance information; medical information; disability information; medical correspondence or medical bills;
- Employment information, such as employment status; employment history/records; job title; certifications; disciplinary records; or self-owned business information.
How do you use my personal data?
Workforce Services uses your personal information to verify your identity and determine eligibility for its public benefits, employment security and other assistance programs. Workforce Services may also collect personal data related to employment within the state of Utah. The data that Workforce Services collects is generally considered private, controlled or protected under the Government Records Access Management Act (“GRAMA”) and may be protected by other federal and state laws and regulations.
What happens if I do not want to provide my personal data to Workforce Services?
If you refuse to provide the personal data that Workforce Services needs, you may not be able to receive public assistance benefits, unemployment benefits or access any other services that Workforce Services provides.
Who do you share my personal data?
Workforce Services may also share personal data with federal governmental agencies, other state governmental entities, educational institutions, community partners, and contractors under written agreements and as permitted by federal and state laws. DWS does not sell the personal data of any individual.
What are my rights to my Personal Data?
You can request access to your personal data or user in writing or request to correct or amend the user’s personal data or user data. Please send your request to DWSPrivacy@utah.gov.

Workforce Services does use website tracking technologies
The website also uses website tracking technologies, such as cookies and beacons. The website tracking technologies collect the customer’s IP address; browser type; the date and time of website access and which portions of the website were visited. The intended purpose of this data is to track how many customers are accessing our website and what portions of the website they are using. Workforce Services does not share the tracking technology data with any other entities.
Changes to the Terms of this Notice
Workforce Services can change the terms of notice as needed or when the law changes. You can find any new changes here at this link, or a new copy can be provided to you upon your request.

At-Risk Government Employee
If you are an At-Risk Government Employee as defined in Utah Code 63G-2-303(1), please request the Department's "At-Risk Government Employee" form by emailing DWSPrivacy@utah.gov. The Privacy Office will send you the form by secure email. Please return this form with the required signatures using this secure email.
