Purpose
In October 2021, the Utah State Department of Health and Human Services received COVID-19 funding from the Federal Epidemiology and Laboratory Capacity for Infectious Diseases (ELC) Program. A portion of this funding has been passed to the Department of Workforce Services, Office of Homeless Services (DWS-OHS) from the Utah State Department of Health and Human Services (DHHS). This competitive grant program, administered by DWS-OHS, is intended to provide funds to homeless providers to detect and mitigate the spread of SARS-CoV-2 (COVID-19) and other infectious diseases in homeless service sites, other congregate living facilities, and encampments.
DWS-OHS is requesting grant applications for projects that provide services to homeless persons such as monitoring infectious disease spread, testing for infectious disease, and quarantining positive cases who are within homeless service sites or encampments located in a specific health district, as specified by the Utah State Department of Health and Human Services.
Eligibility Requirements
Minimum Requirements
Projects must be within the State of Utah. In order to be considered eligible for this grant, an organization shall:
- Apply for and provide services within the specific health districts as listed in the Program Services section above.
- The organization must have a demonstrated history of providing infectious disease mitigation and detection services.
- The organization must coordinate with DWS-OHS and the DHHS for the purposes of responding to cases and outbreaks of infectious diseases.
- Programs must provide services for infectious diseases, specifically COVID-19 detection and mitigation.
- The organization must provide services to at least one of the following sites: homeless service sites, encampments, or congregate living facilities.
- The organization must provide a completed application. Incomplete or late applications will not be considered.
Additionally, DWS-OHS will consider the following criteria when reviewing applications:
- Project sustainability of services beyond the funding source.
- Utilization of infectious control practices in homeless services facilities.
- Utilization of infectious disease prevention measures in encampments.

Addendum
August 8, 2022: The RFGA has been updated to include a more thorough scoring rubric along with some grammatical corrections to Appendix B - Grant Narrative. The Letter of Intent deadline has been extended to August 12, 2022, 5:00 PM, MDT.
