Fee Waivers

During the 2021 General Session, legislation was passed (HB0352) broadening the entities eligible to verify that an individual is experiencing homelessness in order to qualify for waiving fees associated with obtaining an official Utah ID or birth certificate. Under the new law, written verification may be provided by homeless service providers verified by the Department of Workforce Services (DWS). 

The Office of Homeless Services (OHS), which is overseeing the approval process for DWS, will consider all agencies who are participating in the Homeless Management Information System (HMIS) or receive State Homelessness Funding to be approved to verify homelessness.

Other homeless service providers may apply to become an approved provider if the organization is a 501(c)(3), local or government entity that serves individuals experiencing homelessness.

In addition to the fee waiver form, an application for a Utah ID or birth certificate must also be submitted to the appropriate agency.
